this form to be considered for a
presentation. (Please be sure to press
the SUBMIT button to send -- Do not mail or fax
the form itself)

principles apply to all proposals:

proposals must be submitted using this form.
Accepted formats are:
- MS Word (.DOC)
- Portable Document Format (.PDF)
- PostScript (.PS)
- Rich Text Format (.RTF)

Make sure that your File
Name only contains the following characters: A-Z, 0-9.
1. NO dashes, spaces, underlines,
or special characters. ONLY A-Z and 0-9
characters.
2. File extensions are required for a successful upload.

communication will be with the Primary Contact
person who is responsible for communicating with
the presenters of that session.
